Kebijakan Privasi IMMA

Kebijakan Privasi ini akan melingkupi penjelasan mengenai bagaimana informasi pengguna IMMA (“PENGGUNA”) dikumpulkan, digunakan, dan dijaga kerahasiaannya oleh Rumah Sakit Immanuel Bandung dengan tetap menghormati Hak PENGGUNA dalam menggunakan Aplikasi IMMA (“APLIKASI”) KAMI.

Lingkup Kebijakan Privasi
1. APLIKASI dan rumah sakit – rumah sakit (termasuk Dokter didalamnya) yang tergabung dalam Rumah Sakit Immanuel Bandung dikelola oleh KAMI.
2. Kebijakan Privasi ini mengatur penggunaan dan perlindungan informasi yang diberikan oleh PENGGUNA ketika menggunakan layanan APLIKASI.
3. Dengan menggunakan APLIKASI, maka PENGGUNA dianggap telah membaca Kebijakan Privasi ini dan menyetujui mekanisme pengumpulan, penyimpanan, dan penggunaan data pribadi PENGGUNA sebagaimana diatur dalam Kebijakan Privasi ini.
4. Apabila KAMI meminta PENGGUNA untuk memberikan informasi ketika menggunakan APLIKASI, maka informasi tersebut itu hanya akan digunakan sesuai dengan Kebijakan Privasi ini.
5. KAMI berhak untuk sewaktu-waktu mengubah, menghapus dan untuk menerapkan ketentuan baru Kebijakan Privasi ini. PENGGUNA diharapkan untuk memeriksa halaman ini secara berkala untuk mengetahui perubahan tersebut. Dengan menggunakan layanan APLIKASI setelah terjadinya perubahan tersebut, PENGGUNA dianggap telah mengetahui dan menyetujui perubahan-perubahan ketentuan pada Kebijakan Privasi ini.
Registrasi
1. PENGGUNA diharuskan melakukan pendaftaran dalam APLIKASI untuk dapat menggunakan APLIKASI berikut dengan fitur-fitur yang ada didalamnya.
2. Untuk melakukan pendaftaran dalam APLIKASI, PENGGUNA harus memberikan informasi yang KAMI perlukan sebagaimana tercantum pada Butir C di bawah ini.
Data Pribadi
1. KAMI mengumpulkan informasi yang diberikan PENGGUNA saat PENGGUNA membuat akun dan profil, mengirimkan surat elektronik / surel / e-mail kepada KAMI atau meletakkan informasi atau konten lain di APLIKASI.
2. Informasi mengenai identitas diri yang wajib diisi oleh PENGGUNA saat membuat akun di APLIKASI adalah:
  1. Nomor rekam medis
  2. Nomor telepon genggam
  3. Nama Induk Kependudukan
  4. Alamat surat elektronik / surel / e-mail
3. KAMI juga berhak mengumpulkan informasi ketika PENGGUNA menggunakan APLIKASI, termasuk tetapi tidak terbatas pada: lokasi/ koordinat GPS (jika PENGGUNA mengaktifkan fitur ini), Internet Protocol (IP) address (jika menggunakan browser), sistem operasi, jenis browser, alamat APLIKASI yang merujuk dan aktivitas PENGGUNA di APLIKASI. PENGGUNA dapat mengaktifkan atau menonaktifkan layanan pengenalan lokasi saat PENGGUNA menggunakan APLIKASI.
4. KAMI berhak untuk melakukan verifikasi langsung kepada PENGGUNA atas informasi mengenai data diri yang telah disampaikan PENGGUNA melalui APLIKASI.
5. Dengan menggunakan APLIKASI, PENGGUNA menjamin bahwa informasi yang PENGGUNA berikan adalah akurat dan benar.
6. pabila informasi yang PENGGUNA berikan tersebut ternyata tidak benar, maka KAMI tidak bertanggung jawab atas segala akibat yang dapat terjadi sehubungan dengan pemberian dan penggunaan informasi tidak benar tersebut
Data Pengunjung
APLIKASI akan mengumpulkan informasi tentang penggunaan, seperti jumlah kunjungan dan aktivitas pengunjung. Data ini dikumpulkan dengan tujuan meningkatkan kenyamanan PENGGUNA serta pelayanan KAMI.
Penggunaan Informasi
1. KAMI menggunakan informasi PENGGUNA untuk:
  1. Menyediakan informasi kepada PENGGUNA sesuai permintaan PENGGUNA berkaitan dengan pelayanan KAMI pada masing-masing fitur di APLIKASI.
  2. Memberikan informasi/mengirimkan e-mail secara berkala kepada PENGGUNA bila terdapat suatu penawaran dan/atau promosi atas produk- produk baru, penawaran khusus, atau informasi lain yang menurut KAMI diperlukan oleh PENGGUNA
  3. Menginformasikan kepada PENGGUNA mengenai setiap perubahan dari aplikasi, pelayanan dan produk KAMI.
  4. Menggunakan informasi PENGGUNA untuk menghubungi PENGGUNA dan mengingatkan mengenai penjadwalan Dokter yang dibuat melalui APLIKASI.
2. KAMI menjamin bahwa data dan informasi yang diberikan PENGGUNA bersifat rahasia dan tidak akan disebarluaskan. Untuk mencegah akses tidak sah, KAMI melakukan tindakan yang wajar dalam pengamanan isik, elektronik, dan prosedur manajerial untuk melindungi informasi PENGGUNA.
3. Informasi kesehatan PENGGUNA hanya akan digunakan sehubungan dengan pelayanan APLIKASI beserta tinjauan medis Dokter dan KAMI hanya akan mengumpulkan informasi yang dibutuhkan untuk menjalankan pelayanan APLIKASI.
4. KAMI menyimpan informasi pribadi PENGGUNA selama informasi tersebut berhubungan dengan tujuan-tujuan yang dibutuhkan untuk kepentingan pelayanan KAMI kepada PENGGUNA. Kerahasiaan informasi pribadi PENGGUNA tentunya dianggap rahasia dan tidak akan disebarluaskan kecuali apabila diwajibkan oleh peraturan perundang-undangan yang berlaku.
5. Dengan menggunakan APLIKASI, PENGGUNA dianggap telah menyetujui untuk mengijinkan pihak ketiga tertentu untuk menggunakan data PENGGUNA untuk memberikan PENGGUNA informasi mengenai barang dan jasa yang PENGGUNA perlukan.
Akses dan Perizinan Pada Perangkat PENGGUNA
Untuk perangkat Android dan iOS, Aplikasi akan membutuhkan permission / izin akses untuk:
1. Akses Internet & Network State
  Aplikasi membutuhkan koneksi internet, sehingga Aplikasi memantau kondisi internet & network PENGGUNA untuk dapat memberikan notifikasi, jika terjadi lost-connection sewaktu waktu yang mengakibatkan Aplikasi tidak dapat berjalan (secara optimal).
2. Akses Kamera
  1. Aplikasi memerlukan PENGGUNA untuk menyertakan foto pada beberapa bagian dari Aplikasi, PENGGUNA dapat menggunakan Kamera sebagai salah satu media untuk dapat mengambil gambar.
  2. Pemberian akses pada Kamera pada perangkat berfungsi bagi PENGGUNA untuk mengambil gambar dalam memilih foto profil PENGGUNA, foto kartu identitas, video call pada fitur Telekonsultasi dan foto dokumen pendukung pada Telekonsultasi
  3. Pemberian akses Kamera pada Perangkat juga akan memberikan akses "READ_EXTERNAL_STORAGE" & "WRITE_EXTERNAL_STORAGE" pada perangkat PENGGUNA untuk Aplikasi mengolah foto, gambar, video (read & write atau create file) yang diambil dari Kamera perangkat PENGGUNA menggunakan directory external storage secara temporary (file hasil olahan tersebut akan hilang sendirinya).
3. Akses Foto, Galery Dan Files
  1. Aplikasi memerlukan PENGGUNA untuk menyertakan gambar dan/ atau dokumen pada beberapa bagian dari Aplikasi, PENGGUNA dapat melihat pada gallery dan/atau file browser untuk mencari foto, gambar atau files terkait yang dibutuhkan.
  2. Aplikasi memerlukan PENGGUNA untuk dapat menyimpan data-data yang di generate dari sisi Aplikasi maupun aplikasi pihak ketiga yang terintegrasi dengan Aplikasi.
  3. Pemberian akses pada foto, galeri dan files berfungsi bagi PENGGUNA untuk memilih foto profil dan/atau memilih dokumen pendukung yang dibutuhkan dalam Telekonsultasi dan/atau pendaftaran pelayanan kesehatan di Rumah Sakit.
  4. Pemberian akses foto, galeri dan files pada Perangkat juga akan memberikan akses "READ_EXTERNAL_STORAGE" & "WRITE_EXTERNAL_STORAGE" pada perangkat PENGGUNA untuk Aplikasi mengolah foto, gambar, dokumen ( read & write atau create file ) yang diambil dari perangkat PENGGUNA menggunakan directory external storage secara temporary (file hasil olahan tersebut akan hilang sendirinya).
4. Akses Lokasi
  1. PENGGUNA dapat melihat permission yang digunakan oleh Aplikasi dan menon-aktifkannya kapan pun melalui menu setting.
  2. Aplikasi membutuhkan data mengenai lokasi PENGGUNA baik secara terus menerus dalam halTouchless checkin, atau secara situasional pada saat PENGGUNA membuka Aplikasi dalam halOur Hospital untuk tampilan rumah sakit disekitar PENGGUNA dan dalam hal Telekonsultasi untuk menentukan lokasi PENGGUNA saat itu.
Koneksi PENGGUNA ke Aplikasi Lain
1. Untuk keperluan referensi PENGGUNA, pada APLIKASI terdapat tautan ke aplikasi-aplikasi lain yang dicantumkan oleh KAMI. Namun, ketika PENGGUNA menggunakan tautan-tautan tersebut, KAMI tidak bertanggung jawab untuk melindungi privasi dan informasi apapun yang PENGGUNA berikan ketika mengunjungi aplikasi-aplikasi tersebut. KAMI juga tidak bertanggung jawab atas konten pada aplikasi-aplikasi tersebut, termasuk dampak yang ditimbulkan akibat mengakses aplikasi-aplikasi tersebut. PENGGUNA harus mempelajari dan membaca dengan hati-hati kebijakan penanganan informasi pribadi yang berlaku di aplikasi tersebut, maka apabila dikemudian hari muncul permasalahan hukum yang timbul akibat mengakses aplikasi- aplikasi tersebut, sepenuhnya menjadi tanggung jawab PENGGUNA dan PENGGUNA membebaskan KAMI dari segala tuntutan, dakwaan dan akibat hukum yang ada.
2. Untuk keperluan transaksi pembayaran, PENGGUNA akan terkoneksi ke aplikasi yang dikelola oleh pihak Payment Gateway KAMI. KAMI senantiasa melakukan upaya yang wajar untuk melindungi informasi yang PENGGUNA berikan untuk keperluan transaksi pembayaran tersebut.
Hukum Yang Berlaku
Kebijakan Privasi ini diatur berdasarkan hukum Republik Indonesia dan PENGGUNA diwajibkan tunduk kepada semua peraturan yang berlaku di Indonesia.
Upaya Pengamanan
1. KAMI senantiasa melakukan upaya yang wajar untuk menjaga informasi yang PENGGUNA berikan kepada KAMI aman dan tidak dapat digunakan oleh pihak-pihak yang tidak bertanggungjawab. Namun demikian, PENGGUNA mengetahui dan setuju bahwa transmisi atau akses internet tidak selamanya menciptakan suatu APLIKASI yang aman dan pribadi, dan karenanya setiap pesan atau informasi yang PENGGUNA kirimkan atau gunakan dalam APLIKASI mungkin dapat dibajak atau diambil oleh pihak ketiga yang tidak bertanggung jawab.
2. KAMI akan menyimpan data pribadi PENGGUNA dalam server database KAMI, baik di dalam maupun di luar negeri, untuk memberikan perlindungan yang wajar berdasarkan penilaian KAMI dari penyalahgunaan atau penghilangan Data Pribadi PENGGUNA.

Privacy and Policy of IMMA

This Privacy Policy outlines how user information of IMMA (“USER”) is collected, used, and kept confidential by Rumah Sakit Immanuel Bandung while respecting the USER’s rights in utilizing the IMMA Application (“APPLICATION”) under OUR management.

Scope of Privacy Policy
1. The APPLICATION and the hospitals (including the Doctors therein) under Rumah Sakit Immanuel Bandung are managed by US.
2. This Privacy Policy governs the use and protection of information provided by the USER when utilizing the services of the APPLICATION.
3. By using the APPLICATION, the USER is deemed to have read this Privacy Policy and agreed to the mechanisms for collecting, storing, and using the USER’s personal data as outlined in this Privacy Policy.
4. If WE request the USER to provide information while using the APPLICATION, such information will only be used in accordance with this Privacy Policy.
5. WE reserve the right to modify, delete, or implement new provisions of this Privacy Policy at any time. USERS are encouraged to periodically review this page to stay informed of such changes. By continuing to use the APPLICATION services after these changes are made, the USER is deemed to have acknowledged and agreed to the updated terms of this Privacy Policy.
Registration
1. USERS are required to register within the APPLICATION to access the APPLICATION and its available features.
2. To complete registration within the APPLICATION, USERS must provide the information required by US as outlined in Section C below.
Personal Data
1. WE collect information provided by the USER when the USER creates an account and profile, sends emails to US, or submits information or other content within the APPLICATION.
2. The mandatory identity information that USERS must provide when creating an account in the APPLICATION includes:
  1. Medical record number
  2. Mobile phone number
  3. National Identification Number (NIK)
  4. Email address
3. WE also reserve the right to collect information when USERS use the APPLICATION, including but not limited to: location/GPS coordinates (if the USER enables this feature), Internet Protocol (IP) address (when using a browser), operating system, browser type, referring APPLICATION address, and USER activities within the APPLICATION. USERS can enable or disable location recognition services while using the APPLICATION.
4. WE reserve the right to directly verify the USER's personal information that has been submitted through the APPLICATION.
5. By using the APPLICATION, the USER guarantees that the information provided is accurate and true.
6. If the information provided by the USER is found to be inaccurate, WE shall not be held responsible for any consequences arising from the provision and use of such incorrect information.
Visitor Data
The APPLICATION will collect information regarding usage, such as the number of visits and visitor activities. This data is gathered to enhance USER convenience and improve OUR services.
Use of Information
1. WE use USER information to:
  1. Provide information to the USER as requested by the USER in relation to OUR services available in various features of the APPLICATION.
  2. Send periodic emails to the USER regarding offers and/or promotions on new products, special deals, or other information that WE deem necessary for the USER.
  3. Inform the USER of any changes to OUR application, services, and products.
  4. Use USER information to contact the USER and provide reminders regarding Doctor appointments scheduled through the APPLICATION.
2. WE ensure that the data and information provided by the USER are confidential and will not be disclosed. To prevent unauthorized access, WE take reasonable measures in physical, electronic, and managerial security to protect USER information.
3. USER health information will only be used in connection with the APPLICATION services and medical reviews by Doctors. WE will only collect the information necessary to operate the APPLICATION services.
4. WE retain USER personal information as long as it is related to the purposes required to provide OUR services to the USER. The confidentiality of USER personal information is considered secure and will not be disclosed unless required by applicable laws and regulations.
5. By using the APPLICATION, the USER is deemed to have agreed to allow certain third parties to use USER data to provide the USER with information regarding goods and services the USER may need.
Access and Permissions on USER Devices
For Android and iOS devices, the Application will require permissions/access for:
1. Internet & Network State Access
  The Application requires an internet connection, allowing it to monitor the USER's internet and network conditions to provide notifications in case of a lost connection, which may affect the Application's optimal performance.
2. Camera Access
  1. The Application requires the USER to include photos in certain parts of the Application. The USER can use the Camera as a medium to capture images.
  2. Granting Camera access on the device allows the USER to take photos for selecting their profile picture, identity card photos, video calls for Teleconsultation features, and supporting document photos for Teleconsultation.
  3. Granting Camera access on the device also provides "READ_EXTERNAL_STORAGE" and "WRITE_EXTERNAL_STORAGE" permissions, enabling the Application to process photos, images, and videos (read, write, or create files) captured using the USER's device Camera. These files are temporarily stored in the external storage directory and will automatically be deleted afterward.
3. Photo, Gallery, and File Access
  1. The Application requires USERS to include images and/or documents in certain parts of the Application. USERS can browse their gallery or file browser to search for the necessary photos, images, or files.
  2. The Application requires USERS to store data generated by the Application or third-party applications integrated with the Application.
  3. Granting access to photos, galleries, and files enables USERS to select profile photos and/or supporting documents needed for Teleconsultation and/or healthcare service registration at the Hospital.
  4. Granting access to photos, galleries, and files on the device also provides "READ_EXTERNAL_STORAGE" and "WRITE_EXTERNAL_STORAGE" permissions, allowing the Application to process photos, images, and documents (read, write, or create files) retrieved from the USER's device. These files are temporarily stored in the external storage directory and will automatically be deleted afterward.
4. Location Access
  1. USERS can view the permissions used by the Application and disable them at any time through the settings menu.
  2. he Application requires USER location data, either continuously for features like Touchless Check-in or situationally when the USER opens the Application for features like Our Hospital to display nearby hospitals and for Teleconsultation to determine the USER's current location.
USER Connections to Other Applications
1. For USER reference, the APPLICATION contains links to other applications included by US. However, when USERS access these links, WE are not responsible for protecting the privacy or any information provided by USERS while visiting those applications. WE are also not liable for the content of these applications, including any consequences resulting from accessing them. USERS are encouraged to carefully review the privacy policies applicable to those applications. Should any legal issues arise from accessing these applications in the future, USERS bear full responsibility and release US from all claims, demands, and legal consequences.
2. For payment transactions, USERS will be connected to an application managed by OUR Payment Gateway. WE consistently take reasonable measures to protect the information provided by USERS for payment purposes.
Applicable Law
This Privacy Policy is governed by the laws of the Republic of Indonesia, and USERS are required to comply with all applicable regulations in Indonesia.
Security Measures
1. WE consistently take reasonable measures to keep the information provided by the USER secure and prevent its misuse by unauthorized parties. However, USERS acknowledge and agree that internet transmission or access does not always guarantee a secure and private APPLICATION, and as such, any message or information sent or used within the APPLICATION may be intercepted or accessed by unauthorized third parties.
2. WE will store USER personal data on OUR database servers, either domestically or internationally, to provide reasonable protection based on OUR assessment against the misuse or loss of USER personal data.
